Blood pressure and blood sugar readings are most useful when someone is watching for changes. With Remote Patient Monitoring, the readings you take at home are sent automatically to our care team, with no notebook to bring to your next appointment, no app to set up, and no home WiFi required.
Our care managers monitor your readings for changes and patterns that may need attention. If something looks concerning, we follow up with you, and clinically significant results are escalated to Dr. Ayesha Kanwal.
Remote Patient Monitoring (RPM) is a technology-enabled monitoring of important health data from your home. You take a reading with a device our office provides. The device transmits it to your care team on its own. We review it, look for changes, and follow up when something needs attention.
It is a straightforward idea with a real effect: problems get caught in the weeks between visits, instead of at your next appointment.
RPM is best suited to patients whose conditions are managed by the numbers where a trend over several weeks tells us something an occasional in-office reading cannot.
Our office provides the device. You do not buy it, and you do not need a smartphone, a tablet, or home WiFi to use it.
An upper-arm cuff monitor for patients tracking hypertension. Put on the cuff, press the button, and the reading is transmitted on its own.
A connected glucose meter for patients tracking blood sugar. Each reading is sent to your care team automatically.
For patients tracking both, a single device measures blood pressure and blood glucose, one device to charge, and one device to keep track of.
This is the part that makes RPM worth doing. Your readings are not just stored; they are watched.
If your device stops working, gets damaged, or is lost, call us. We will deactivate the old device and issue a replacement, and we will walk you through setup again if you want it.
When your monitoring period ends, you simply return the device to our office. There is nothing to ship and no equipment to keep track of afterwards.
Remote Patient Monitoring is covered by Medicare Part B and by many other insurance plans. Standard deductibles and coinsurance may apply depending on your coverage. The device itself is provided by our office; you are not purchasing equipment.
We verify your benefits and explain your expected cost before you receive a device.
No. The device we provide sends your readings on its own using a built-in cellular connection. You do not need a smartphone, a tablet, home WiFi, or an app.
Usually a few times each week, though the exact schedule depends on your condition and what Dr. Kanwal recommends. You will be given a clear schedule when you receive your device.
Nothing punitive. If your readings stop arriving, a care manager calls to check in. Missed readings are common, and following up on them is part of the program rather than a problem with it.
Take the reading again to confirm it, then call our office. If you have symptoms that worry you, seek emergency care rather than waiting for a call back. Our team also reviews readings as they arrive and will contact you if something needs attention.
If you are tracking both blood pressure and blood glucose, we usually provide a single combination device rather than two separate ones. Dr. Kanwal determines what monitoring is appropriate for your condition.
No. The device is provided by our office at no purchase cost to you, and you return it when your monitoring period ends.
Yes. RPM pairs with either PCM or CCM. Your readings feed into the same care plan and are reviewed by the same team. We will confirm your coverage for both before you enrol.
